Hi Chandoo,
U will get the PF withdrawl form from EPFO
Download 2 forms.
1. Form 19: Employees' Provident Fund Scheme, 1952
2. Form 10-C Pension: Employees' Pemsion Scheme, 1995
If u have worked in a company for more than 6 mnths, den submit both the forms, otherwise submit only Form 19. As employee doesn't get the pension amt, if he/she has worked in a company for less than 6 mnths.
