I have learn that data gained from an employees' exit interview is a very powerful tool.My learning-that it shouldn't be just left in the form,data should be analysed and a report to should be presented to the employees' respective Manager/skip level manager/can also b shared at a quarterly or annual basis to the business head.Helps identify problem areas within the system.
Currently working on an exit interview analysis format..any suggestions are welcome.

hi,
as a new user I also want to share an Exit interview form with you, it contains only a few open questions, so it is easy to prepare reports from this document.
(Maybe it is easier if you convert it into Excel format for a report)
