Dear all,
I am assisting my organisation develop a change management strategy. we have conducted some workshops and identified some key issues to be addressed. This now needs to be developed into an action plan that the organisation can use over the coming months. I am wondering if anyone has some suggestions on formulating the action plan and structuring an overall strategy, e.g. whether in themes of issues or stages of action etc... Maybe someone has a template that they've used before?
Also, I'm keen to get a hold of some case studies where change was implemented successfully, particularly in health care and with nursing staff.

Pls change the idea of using existing templates
If you are comfortable using a template or workflow, pls use FMEA chart after customizing suiting to the change you would like bring in to the organization. Every change you suggest can be taken up as individual project with start date, end date, specific tasks assigned to people etc
