I had a look at your form . Even i a MBA student and also making an Interview Evaluation form .
As far as i have come to know is that the interviewer want a form that is short and very precise as they know what they want in a person for the position. So i would tell you to give a thought abt :
1. Qualification: as it will be present in his resume and the evaluator has to just match the qualification with the requirements.( e.g MBA from XLRI for HR post is excellent )
2. Interviewer's name and signature should come together in the end
3. How can you evaluate the analytical/ technical ( if it is needed)/ persuasiveness/ enthusiasm.- these have to be very very clearly defined or shuld be taken out coz usually the evaluater leave these blocks blank ...
4 make area for comment and recommendations ...
5 for the status: u can include
select to offer
shortlist to final
shortlist for next level
I really liked the idea of keeping the strenghs and weakness option ...
