I have to design an appointment letter to the role of 'Quality Management Representative'

I think you are talking about appointing a Management Representative for your QMS to get ISO certification. One of your senior employees would get appointed as MR in addtion to their current responsibilities. If you need a format for that appointment, pl find one below:

Mr. ______________________, (current designation), is hereby appointed as Management Representative (MR) for establishing, documentation, implementing and maintaining ISO 9001 and ISO 14001 systems in our organization. His responsibility and authority included

a. ensuring that processes needed for the Quality Management System, Environmental Management System are established, implemented and maintained,
b. reporting to top management on the performance of the Quality Management System, Environmental Management System and any need for improvement,
c. ensuring the promotion of awareness of customer requirements throughout the organization, and
d. Liaison with external parties on matters relating to the Quality Management System, Environmental Management System.

He will perform activities of Management Representative in addition to his existing duties as (current designation).
