Format is good but is based on HOD's review only, factors like Linency,HALO,Recency may produce a effect on this. In ur case I think a format for employee feed on HOD's review will complete it and excercise will be effective.

I agree, this is too generic, there is more that goes into competency identification than what is mentioned in the form. Since line managers need to understand the process , it has to be more elabotare and specific to context.
