It has observed that you are not coming on duty from 15-11-2012. You have remained absent from duty without information and approval. You are aware that such unauthorized absence amounts to misconduct as per the rules of the Company.
You are therefore intimate to call upon the report for duty within 05 Days from the receipt of this notice and explain the reasons for your unauthorized absence.
