This is J. Subramanian, a new member of this forum. I am managing the HR Department of a new startup. We are in the process of putting up a format for availing leave (all types of leave). I have some basic structure in mind. I shall be happy and thankful if any of our forum members can show the format followed in their respective companies (even if it is general, it is welcome).

Apologies for the very delayed response. Due to certain other preoccupations, I was not able to immediately send it to you. I am sending the leave format I have designed after taking inputs from various sources, including the one given by you. Feel free to offer your comments on the format.

Hi SubramanianJ,

You took a long time to respond. Anyway, good to hear from you. I have gone through the formats you have designed; they are quite impressive. Although I would like to offer you some suggestions:

For PL - You can include the LTA also in the form if the company has a policy of providing LTA. Once a year when LTA is claimed by the employee, they ask for PL also. It would be good to include that.

For CL - This format is very comprehensive :D. One change you should make is to put the column "Employee Signature" at the bottom right corner.

I really appreciate your efforts. Keep the ball rolling.

I once again apologize for the delay in replying to your earlier communication. I thank you very much for your excellent feedback. Regarding LTA, our company does not have one at present. But as suggested by you, we have to take into account all future requirements when preparing formats and formulating rules. I shall surely do as suggested by you. Regarding CL too, I shall do as pointed out by you. Once again, thanks for your valuable comments.

Can you please tell me in detail the types of leave applicable in a firm? As per my knowledge, there is SL, CL, and PL. Are there any other types of leave apart from these? Also, are preferential leave and annual leave the same?

In our firm, we have a total of 48 days of leave, out of which I have only availed six days. Can I avail encashment for the remaining 40 days of leave? Please advise.
