I am attaching hereby the format which may be useful to you all. You can make corrective changes as per your organizational requirements.
Just give me some more time so that I am able to attach additional competency mapping assessment format used as the time of interview.

Let me know the suitability and usefulness for these formats.

Had a couple of queries on the attached sheet, if you can spare some time to address these queries, I will be highly obliged:
1) What is the formula for calculating Stability Index used by you.
2) Do you have a test for checking individual's Planning & Organizational skills or it's the way an inviewer evaluates???

For stability index the formula is simple as
no of years of experience / no of organizations he has worked with
As far as checking planning and organizational development, we are not having any formula. But since we are sales and service division we asked the questions related who a person plans his day, visits to customers, how spend his time (he can give brief about time / day spend), a simple problem can also be given (like case study) with no of activities and we ask him to plan such things, etc.
After joining we have CRM software in which he has to make entries and day planned and spend.

Well the formula remains the same for us too.
Basically the evaluation for the 2nd query is subjective, that's good. It's a good way of evaluation. I will definietly try and implement the same for our industry.

Small Suggestion if you can add in the sheet its Education skill in %, Work skill in %, other knowledge sikll in %. Its only my Opinion if I may tell anything wrong sorry...........................
