I understand the fact that while dealing with other companies HR, one has to be professional while dealing with them. But some times we simple approach can be uesful.
You can always know the PA of other companies by addressing the following question:
'What is your basic approach while dealing with PA/Competencies in PA forms?'

I agree with u that ratings talks about peformance not the form.
But we are actually want to change our whole process including form also.
So we are requiring some questions we can put in front of any organisation HR person, so that we can get to know their process.
