EPF Calm Form 13 Rejection-reason

Hi
I have filed FORM 13 to transfer my previous EPF account to present EPF account, but the application rejected due to "3A/6A NOT SUBMITTED BY EMPLOYER"
what can i do next? how to transfer my account
