bonus letter format

Hi,

One of our employees have denied to accept a bonus amount that was being awarded to him, due to some personal reasons. Now the management wants me to come up with a short write up which will document the fact that the bonus was offered to him in recognition of his good work in a particular project, and that he has declined to accept it.

Can you please suggest what should be the relevant wordings for a letter like this?
