Dear Gauri,
It is hereby clarified that we must use those forms which are provided in the State Rules of the particular Act in question. If the appropriate Govt. is central govt. then adhere to the Central Rules otherwise plz go through the State Rules in which your unit is situated. Form F is the nomination form under the PG Act/Rules.

Dear Member,
Welcome to CiteHR.
This is to inform you that none of the enactments provide us the forms. It is the Rules which provides us different forms. Therefore first decide which of the central or state Rules of Gratuity & Minimum wages Acts are applicable in your case & then go through those Rules to have forms under these Acts.
