Exactly there are many types of appointment letter formats.
But for basic elements let me tell u that there must be specify the following informations :

1. Name of Post
2. Joining Date
3. CTC
4. Probation Period
5. Leave Details
7. Confirmation & Termination conditions
8. etc.
