Default Rating Scale Performance Appraisal

Hi Everyone!
I've recently joined a company which is about to implement performance appraisal system. Can someone please tell me more about the grading system and give me formats of appraisal forms for the same. I will not be able to implement the 180 degree or 360 degree appraisal system because it would be too complicated at this level.
