Recently one employee joined our company who has previously worked in a firm (that firm is now shut down) with ESIC facility. He told that he lost his ESIC identity card but knows his Number. The quetion is can we activate the same number by applying for a duplicate ESIC Card for the lost card ? Kindly direct us what is the procedure to be followed ?

Hi,

You can do the needful at your end, for this you have to visit your ESIC local office there you can have all the details and procedures.

You have to submit new Form 3 in respect of the insurance no. Automatically the number will be shifted in your code.

Pls keep all the correspondence of this regard for future reference because any time you will need these things to show at the time of inspection and all that.
