Pls find attached format of ESI Challan. ESI Challan is the challan to submit Monthly Employee and Employer's contribution. It is to be submitted on or before 21st of every month. It is submitted in any SBI Branch.

4 copy of pf challan
5 form 2 (nomination form)
6 with covering letter

pf challan have quarduplicate copy. Can any one advice me which colour have to retain by employer & State bank.
