Hi,
Both the template/formats are fine. Different kinds of leaves should have specific reason to be mentioned. It should be better to indicate the following in the leave application. Leave address/contact numbers. the person to whom the charge is being handed over during the absence of the employee. In senior/responsible positions, the incumbent should hand over charges to a designated person so the flow of work during the absence of the employee/officer will continue.

It would also be better from the part of the organisation to issue an order stating the sanction of leave the concerned employee/officer. Thus a proper record of the leave can be maintained.

in dubai or in any gulf country, as there are more expatriates, you need to consider the following

1. include provision to select the travel plan. entitlement to be put by the admin.
2. some companies prefer to settle the payment due.
3. in some cases, you have to make provision for the replacement to be seeked by the departmental head.
4. handing over or taking clearance is very important.
5. type of leave. emergency / annual / haj, etc.
