Default Warning Letter

Hi,

I need to draft a warning letter to a staff who not in his duty place during his duty.

Can i have any sample of letter to draft this letter?

Warning Letter in case of Coming late after break without information


21st June 2007

Mr.

Sub. – Warning Letter

Mr.

It has been observed that you you are logging late after break time up without prior permission of the concerned authorities, resulting in willful insubordination and gross negligence of duties, in your capacity as designation.

Logging late after break time up yourself to duties without prior intimation is a misconduct for which you are making yourself liable for necessary action.

You are hereby warned to refrain from such activities; failure to do so shall invoke appropriate action.

You are further advised to submit a written explanation on your logging late as soon as you receive this letter.

Kindly treat this as very urgent.

Again in need of your help

Please provide me the format of warning letter for the personnel who are not achieving their goal/ target. since I have to issue this warning letter to some to our employees who are not achieving their targets since last one month.
