Help required on Assessment Sheets

Dear HR Friends

Can anyone help me in designing a Assessment Sheet for a Soft Skills training programme spanning 24 Weeks.

I want the following information:

- Initial Assessment Sheet
- Phased Assessment after every 4 Weeks
- Final Assessment Sheet.

I not only need help in designing these forms but also want to know what are the things to be kept in mind while designing the same.

Hi Sujatha

I am actually not able to understand what assessment sheet you want.
Assessment sheets are made based on our topics.
Initial assessment sheet would consist some general things and the purpose of this is to assess the level of the subject knowledge of the person so that you can desighn your training programs keeping their requirement in mind.
Phased assessment is carried to assess the grasping capacity of the participants and the final assessment would be to assess the level of understanding or knowledge gained once the training is over.

You can also conduct an effectiveness test after a few months of training to assess the effectiveness.
