Hi,
A Performance appriasl form should cover the followng 3 aspects of a role.
A) On Performance.
B) On Potential.
C) On Soft skills/ attributes required in the role.
Also, the form comprises of 3 parts, the form is first filled by an employee, the he is rated on the same parametrers by his manager/ supervisor.
Then in the third part the ratings are discussed between the Manager, the functional head the business HR.
