We make questionnaire to determine various factors which could be a reason for employee de-motivation. Providing motivational quotes is helpful only to develop intrinsic motivation to some extend.

First of all I need to know the purpose of the questionnaire. are you a student and need to subit an assignment / project or you are an working professional and need to do a survey. If professional then please mention about the industry and who would be the probable participants.

Dear Sonam, What course are you pursuing and where will you be using the questionnaire? Have you done any literature survey and decided on the aim and objectives of your study?

Dear Sonam As far as I guess you want questionnaire for your BBA or MBA thesis. So, your questionnaire should be consist of minimum 20 question. It is better to include all close ended questions because they are easy to analyze. Questions should cover all the factors that can increase or decrease employee motivation. for e.g salary packages, fringe benefits Company HR policy for allowing leave, grievances handling, supervisor behavior etc
