You would need to issue a letter stating that the employee name performance was satisfactory in the probation period therefore your dept. is changed to abc dept. and designation will be xyz.
attached the new salary structure and get signed from employee as well as authority after that you can change the dept.

attached the new salary structure and get signed from employee as well as authority after that you keep that original copy and give another copy of same document to employee.
