Are you talking about an establishment in Malaysia?

If it is in India and the employee you are referring falls in category of "workman" may pl. check your standing orders. Since warning amounts a punishment (though a minor) one needs to follow certain procedures like informing the employee about his absents, issuing show cause, conducting an enquiry (check your SO whether it is necessary even to inflict minor punishment?).

There after you can draft as under

You have remained unauthorizedly absent from your duty on following days.

You’re this act amount to misconduct as per clause...of standing orders applicable to you. Remaining habitually absent without prior permission adversely affects companies business. This act of yours calls for punitive action, however looking towards your past records management has decided to offer you an opportunity to correcct upon. If you do not desist from remaining unauthorizedly absent management will be compelled to take punitive action as deemed fit.

HOD and Disciplinary authority

If the employee is not a workman and falls in executive cadre, after informing him that remaining absent is a indiscipline and you are advised to correct upon the same . Upon repeat of the absenteeism thereafter you may issue him a warning or a note under progressive discipline procedure.
