I am working on Performance Appraisal and preparing appraisal forms as well
Need your inputs on the below:
-- How can be effective performance appraisal conducted (For eg quarterly, bi annually or annually)
-- What are the mail points to be added in appraisal form

Hi Sharvari
Performance Appraisal should be a continuous process.
Appraisals can be conducted monthly, quarterly or yearly - it depends on how big is your organization and how much importance does the management gives for performance measurement.
You can go for yearly appraisal. But my suggestion is that each month make sure that you keep track of performance of employees. You can go for critical incidents method where supervisor notes the critical incidents of his subordinates in the dairy which he can use it during the yearly appraisal time.
Yearly performance appraisal can be on three parameters
1) Output based
2) Behaviour
3) Attendance
