Really helpful Questionnaire but it would be more helpful if you had posted/attached the analysing tools as well so that we would know the technique to bring and use the outcome of/from it.

Hi,

Quiet a well thought out questionnaire.
But i noticed that it is conducted by an internal Analyst, would it be possible if you could post a questionnaire conducted by an external Analyst for a health providing organization relating to the effectiveness of Management information system.
