Kindly clarify your position in the organisation and the type of organisation you are working for. Also clarify whether you are training your own staff or you are training persons from various other organisations. What are the kinds of training you are offering and what is the level of participants? What is the duration of training? Is it induction training or on-the-job training or skill-improvement training? The feed back form has to be designed to suit the specific needs. If it is designed in a general way to fit various situations, it may become unwieldy and difficult to collate and process.

Hello Bhumeshwari,
It is important to give details that Shri B Pugazhendi has asked for.
The training feedback provides input for further analysis.
1.0 If it is internal faculty, it reaches him through HR. With the feedback
he would perform better.
2.0 If it is external, the company can take a decision to continue or not
with the same faculty.
3.0 HR can analyze how the same programme has generated different
levels of comprehension and learning amongst the participants.
Many more can be added.
However to meet your immediate need I have enclosed a General Feedback Form.
Hope it would suffice.

From your above post it is not clear whether you have already got the feed-back from the participants or not. In case you have not got them, you can devise a suitable format. It should be designed in such a way that you get the essential feed-back and also able to easily collate and analyse them. It should be simple enough not to strain the faculties of the participants. It should not be too unwieldy that people get bored in filling them up and give some pointless remarks. Before designing the feed-back form we should be clear about the objective of the training programme. The feed-back is just to assess whether the training has achieved the objectives or not. We must also decide when we should take the feed-back - whether immediately after the training or after a certain time gap or both? We should also decide from whom the feed-back is to be taken - from the participants or from their superiors or from both? Keeping these in view you can draft a format and post it here so that it can be fine-tuned by the experts. In case you have already collected the feed-back and require only some suggestions about how to go about collating and analyzing them, you may post a copy of the blank format here so that others can suggest.
