You have to approach the Office of the P F Commissioner nearby to your company and get a set of firms for applying code number of EPF. Further you have to submit the copies of the attendance register, wages register, registration documents from small scale industries, PAN, Lease cum rental agreement, and other related documents specified in the forms.

Pl.go through the website of EPFO where procedure/proforma for Registration is available.
In short, the employer must have 20 or more employees ,and should not be attached with one single Principal Employer.
There is a provision for Voluntary coverage also IN CASE THE NUMBER OF EMPLOYEES ARE LESS THAN 20, proforma for which may also be seen in the website of EPFO.
