Dear seniors,
Please suggest forms to be used for change of occupier position from one director to other director. The old occupier still continues as director. Most urgent.

PF Authority - Form 5A
ESIC Authority - Form 1 - Which is now online in company's ESIC code account
Factory Licence - as per the local government authority (consult into factory inspector office)
Professional Tax - As per Municipal corporation or state government - whichever affects in your state

Dear S. Ganapaty
First of all you please obtain a certified copy of Board Resolution passed by your company’s board in which the decision on change of occupier was taken. Then sand a copy of the same to your area’s director of factories intimating about the change.
Thereafter, please send form no. 4 (number may vary as per UP Factories Rules it is 4) in triplicate.
Just go through the sec. 7 of the Factory Act and you will have an idea about it.
You have to deposit prescribed fee for this (as per UP Rules it is Rs. 5/-) and along with the form + copy of challan of fee so deposited + original Factory Licensee be deposited with the Director of Factories of your area for making the changes after approval.
please note not the every director of the factory can be nominated as occupier only the director who has ultimate control over the affairs of the factory can be nominated so please take precaution in this regards while dealing with the matter of change.
