Yes many a times we play blame-blame Game when any accident happens rather than finding out its root cause and get it rectified.Many a time even immediate causes are mistaken with root causes.

Dear Ramesh,

People has already said "Good Words" about PPT, but I think there is some problem in my PC. I am not able to see the ppt now but I'll check it later. Still I would like to say few words about accident investigation that people maximum time search for a scapegoat instead of searching the root cause. And sometimes people gets confused between root cause & immediate cause (@Hansa nicely said by you). I would like to give one simple example of one LTI.

i.e. One labor got injured while he was using a damaged ladder.

Now report says that damaged ladder is the root cause of accident. But actually this is the immediate cause & we have to think a bit deeper. Please keep "Multi Causation Theory" in mind, while investigating the accidents. We have to ask following question in our mind,

1) Why did the labor use the damaged ladder?

2) Is it the lack of supervision?

3) Did we give proper training to labor?

4) When was the last inspection done?

5) If damaged ladder was identified in inspection, then why it was not tagged "Do not use" or sent for repair?

6) Did we give proper training of tagging to our workers?

There may be some other questions too.After getting the proper answers of above questions, we may come to know about the root cause.

It may be lack of "Supervision" or "Training" or any other cause but not the damaged ladder, which is an immediate cause.

Dear Inderjeet,
Root cause analyses is very crucial to find out the reasons and fix the causes so that CAPA may be established.
Further required training on the subject must be provided to all the employees working on the workplaces. Lack of supervision on the part of the concerned HOD's leads to casualness among the workers who can ignore the safety rules and become the victims of the accidents.
However efforts required from all the HODs and the employees to avoid the accidents, even sometimes we have to take strict disciplinary actions against the defaulters or who ignore the safety rules.....................

Dear All,
Good Discussion on this thread so far.
All said and done it is important to find the real cause. We can follow why why analysis.
Usually the true root cause can be identified with fifth why.
Please note that the Corrective changes as the root cause changes.
