Hi,
here is a very basic outline for a questionnaire for behavioural trainers
NEEDS ANALYSIS INTERVIEW
YEAR 2007
NAME:
DEPT:
DATE:
1. What should be the objectives for the training programme (AOI for the employee?)
2. How does the identified area pose as a problem in performance currently?
3. What are the symptoms observed? (Examples where Skill/Behavior/knowledge is lacking)
4. What are the probable causes? (Attitudes/Behaviors/System/Policy/Non-clarity/Unconscious etc)
5. What will be the training implications?( expectations after a training program)
6. What are the non-training implications?( aspects that a training intervention cannot affect)
7. What situation/problem/issue needs the most attention?
