Yes it is necessary to give such declaration form.If your current establsihment is covered by the P.F act, it is advisable to get your P.f fund transferred to the account of new establishment.

How many months you have spent with them? I mean your employment period with your employer bcz if you have no further planning to go for employment/job; you certainly have to give them a declaration that you are not working somewhere which is really important and there is no harm in it too. You can give them and why it is important and have been asked you to furnish because employee changes job and never informed previous employer about their new employment or for EPF account transfer. And New Employer has to open their new EPF account which is creating a Burdon on Govt/EPF Department. Therefore, it is important to do so.
Why are you taking so much of worries about this? It will cost Rs 40 max to prepare a Affidavit and you can easily furnish this to them. You already have resigned from your position, and guess already have your F&F and all dues along with your Experience/relieving letter.
