Dear Sudipta,
The questionnaire that you are going to design for the employee motivation survey.
1. First you have to make it clear that whether you are planning to do the survey on upper, middle or lower management level employee.
2. After that the question should be mostly closed ended so the analysis can be done easily and accurately.
3. If you want to get better results then do the analysis using SPSS (Stastical Program for Social Sciences)
4. The questionnaire should not be more than 20 or 25 for better analysis purpose.
5. The answer should be measured on a scale like 2 point (yes or no) or 5 points like (Excellent, Better, Average, Poor, Very Poor).
