Dear Friend,
Let us understand your exact need.
1. Whether your Organization is an ISO 9001 2008 certified company?
2. If yes! The company should have been certified by some Certifying body- which means:
3. You should have access to Quality Manual (QM) & Quality System Procedure (QSP)? then read it - you have the right to do so.
4.Then go to the Resource management and look into scope - Human Resources, Infrastructure, Machinery, Equipment, & Work Environment.
5. Formats are designed as per need of the organization but with some generalizations should have been already designed. Follow procedure.
6. In case of HR, competency mapping + skill matrix analysis, the determination of Competency Requirements & Authority & Responsibility, Individual Competency Records
Training Needs Identification, Training Calendar etc has to be managed.
7. Buy for your self a copy of Standards ISO 9001 2008.
Please let me know- about the above then about ingredients for format.

It will be a great thought to implement the ISO 90001:2008, Once you initiated every thing will comes under the mangement control.
hence whatever happens the prcocess remains the steady state position.
ISO 9001:2008 classified by four levels
Leve I - QSM - Quality System Manual
Level II - QSP - Quality System Procedure
Level - III - Skilll Matrix, Standard Operating Procedure, Technical standards.
Level - IV - Control of Records & Documents
Hence it has been clearly explained in ISO9001:2008 standard

Dear Friend,
Did you Purchased the ISO 9001:2008 standards ???
If Yes ... Read Carefully.....then its easy to work on that ... For QSM is nothing but your standards according to your scope.
Standard Clauses from 4 - 8 are mandatory.
QSP is nothing but the way how you maintaining and documenting details with process flow diagrams.
By Knowing your setup and Scope of your Organization can help you more from my side...
Instead of more details u can mail me

The ISO 9001 standard provides a framework of globally recognised principles of quality management, including: customer focus, leadership, involvement of people, process approach to management, continual improvement, factual approach to decision making and mutually beneficial supplier relationships. These are also known as the eight key principals of quality management.
