For that first you need to declare previous employment details, if any, in Form No. 11 to the New/Current employer.

Just Fill up the necessary form and sign that form also attach your updated bank passbook/statement (Account must be with any of the nationalized bank) zerox copy to receive a PF Amount directly credited to your bank account to be given to your New/Current employer.
Claim application in form 19 may be submitted to the concerned Provident Fund Office.

For withdrawing PF, you also need to submit FORM 10C(EMployee Pension Scheme) along with Form 19, so as to withdraw the whole amount of deposited PF (12% of employer's share).
