Kindly pls help me with Effectiveness on Recruitment!

I need to prepare questions on that how to find effectiveness on recruitment, see its like finding out the effectiveness and the finding out the process is right or not for the company. Pls help me, its urgent

I am also doin MBA in HR and right now doin a project on "Recruitment". I am sure you wud have finished ur MBA by now.. can u plz help me with a sample questionnaire..
