You have to fill below documents.
1) Form No 2 ( Renewal of License) :- I marked yellow for you & fill all necessary details
2) Treasury Challan Form :- Ask the fees might by it differ from state to state and I am also attaching(AP challan)
3)Covering letter : Stating to what all documents your are providing & Fee details.
All documents submit to Factories Inspector.
If anything plz let me know.
