You can start by saying it "TO WHOMSOEVER IT MAY CONCERN" and in order to make it as address proof you may start it by stating that so and so employee has been employed with so and so organization at so and so designation since so and so date(give the beginning date).

Else it also depends in case if any address proof is required same can we mentioned that as per employee records,permanent/temporary address of the employee is so and so.
