One of my employee had been in Maternity leave for 3 months (as per ESI act). She is eligible for ESI leave. She came to duty after completion of her ESI leaves. Now she is going to claim ESI amount. They are asking a letter that she joined back to duties after completion of Maternity leave.
If we give that letter from employer side then only they will start other process
Can u please provide a letter format to submit in ESI office to get her Maternity amount

You have to give a joining report in Form 12A , 13,14,15 maternity benefit sickness. Refer ESI Act 1948. Did you asked her to register her ESI card before she went for the maternity leave ? Dis she got the Doctor certificate for the maternity and the discharge certificate from the hospital ?
