Deepali! These are the basic points to be included in an aplication form. You can add more points as per your need and design it in your way.Like you can make a table for qualification, position aaplied for etc...

Hi Deepali,
The application blank or the application form is a very important form which will contain the following informationn as Ms.Sweta has already mentioned in her mail to you.
This is a very important form which gives information about the candidate. Usually this form is given to candidates before the interview commences. This can give you a lot of form right from the post applied for in the organisation to his family background / educational / references / previous employment (which could start from the present to the previous: as when he/ she started their career)
The forms will also depend on the way they are designed to hold more information.In organisations there are application forms given for the candidate as soon as he / she joins & accordingly the information is then transferred to the respective personal file. trust this information suffices.

I think your topic points for new employee recruit. Im not clear with your question... there is a form for employee evaluation, that is used at the time of Appraisal. For knowing about employees .. yo can go for Questionaire(self assessment)

NO I AM PREPARING IT NOT ONLY FOR THE NEW RECRUIT BUT FOR ALL THE EMPLOYEES. I AM STARTING EVERYTHING AFRESH. u CAN SAY, PROBABLY I MAY NOT GET IT FROM PAST RECORDS OR ALREADY COMPILED RECORDS COZ THEY ARE NOT UPDATED , SO I HAVE TO START AGAIN AND I HAVE ONLY PRIMARY SOURCE FOR DATA COLLECTION, NO SECONDARY SOURCE..
