The form 37 of the ESIC is self explanatory. fill up the details of the employee / ESI no. but take care that while submitting the same to the local office your office should be in their jurisdiction . also if there is an ESI no. of the employee which pease mention.

Form 37 is declaration by employer with respect to continuity of employee during insurable contribution period to be submit on demand by local ESI office.
In the ESI Form 37. only 4 requirements are there , these are
1.Name & Address along with ESI Code of Employer.
2. Name and Insurance number of employee.
3. Declaring start date of current contribution period and date of reemployment / re entering the insurable employment of employee.
4. Declaration of payment for not less than of half of the No.of days worked during the last half yearly return which ended on 31st March or 30th September.
So it is very simple. Fill it and submit the same with Local ESI office.
