one suggestion:
analyse attrition at various stages like hiring, training, pilot/ probation period itself and in a year or two.
add parameters like functional area, supervisor/ person incharge etc.
would give plenty of opportunities to make imrovements.

1. Ask how they screen the profile & speak to the aspirant.
2. How they access the aspirants interest towards the job/interview.
3. How they counsel the right aspirant who is not interested in joining.
4. How they will get to know about the aspirants attitude.
5. What is the motivating factor which makes them in achieving the goal.
like this you can ask the questions related to recruitment giving certain options in incase it is a closed questionnaire.
please let me know your suggestion in the above matter.
