Dear Shri Sunil Ji
For withdrawal of your PF dues, you have to forward application to the EPFO through your previous employer.
The EPFO will arrange you payment directly through your bank account. It will not be routed through your former employer. Plz request your former employer to forward it to the EPFO at the earliest and also mark a copy of forwarding letter to you. You may pursue the matter on the strength of this copy with the EPFO after giving them sufficient time of about 45 days or so.
Thanks and regards,

Dear Shri Phani Medavarapu Ji,
An application for PF final withdrawal sent through a gazetted officer or a branch manager of a bank if the employer firm is shut down and is no longer in existence to process and forward the application to the EPFO.
For crediting PF dues, the EPFO needs details of bank accounts of the applicant payee viz. bank account number, name of the bank, branch code etc. If an un-used cheque is attached with the application, the EPFO knows these details without any mistake and it becomes easier to credit the dues to that account.
Thanks and regards.
