Dear srvenkat

Welcome to CiteHR.com !!!

There is no need for a format as the situation/reason may be different for everyone concerned.

Just address your email to the person concerned, who isued the offer, quote the reference of the offer letter; tell the reason and then request them to extend the joining date.

They may or may not extend the date depending on whether the likely date of your joining, suits them or not.

Warm regards.
