The format seems to be too brief. You may include a few accomplishments of the employee as a team member or as an individual if you are keen about his better prospects else where. Please wish him all the best and it is displaying the right culture of the organisation.

Dear,

I guess the employee should not sign on the experience letter. Also the way employee performed his duty has to be mentioned in the letter, highlight the positive factors of the employee as well. Also I believe that it is not the part of responsibility of the company to mention educational qualification of the departing employee. Pls correct me if I am wrong.
