There are standard formats available but it would be wise on your part to devise your own based on your requirement. A few tips
Location : Toilet
Activities (Person responsible - HK employee) :
Cleaning Toilets, Urinals, Wash Basins, mirrors. Mopping floors - to be dry all the time (Decide frequency based on the traffic - people using it)
Providing Toilet Tissues, Hand wash liquid, urinal cubes, room freshner, dustbins, Hand tissues or towels, buckets mug, toilet brush, disinfectants etc
Cobweb removal - Frequency weekly
Activities (Person responsible - Maintenance)
Look for leaking taps, change of lights, non functioning lights, exhaust fans, defective switches, defective door closers etc
The check list will be ticked by the person responsible and verified by yet another supervisor who will check the checker.
The check sheet cited above can be prepared for one week and has to be displayed in the location with a provision for people to include their complaints/ suggestions. These check sheets must reach the administration department alongwith the monthly bill of the HK staff,
You can develop different check sheets for different location in a similar manner depending upon the needs.
The reason why I am not attaching you a standard format is you might miss out certain things which may be relevant in your organisation.
In short there should be two things one the activities and check points. Activities have to be carried out daily (cleaning, mopping etc) whereas the checking can only be a verification. (Are toilet tissues available, exhaust fan is functioning etc).
