I am an MBA student doing my summer project on Employee engagement in organisations. I wanted some help regarding the questionnaire which would be asked to the HR managers regarding their engagement practises.

Engagement Practices can be many fold from different aspects like say for e.g.:

From Salary perspective:
Is the salary competitive when compared to other companies in Market?
What are the additional Monetary and Non-Monetary perks?
What is average percentage hike offered at time of Appraisals?
Paying up the interest portion of home loan or education loan?
Benefits confered for candidates who come from outstation?

From Entertainment Persepctive:
Project Parties?
Team outings?
Sport activity?
Annual trip with family?

From these broad parameters you will have to require the questionarie

I hope this helps.. will give more dope later... will find out from other HR friends....

I did a project on Employee Engagement as part of my Final Project. It is based on Gallups Q12 which is a researched questionnaire, I added 4 questions which are of relevance to the Indian scenario to broaden the scope of my study. The questionnaire may be of help to you.
