why do u need age when you already have an option for date of birth and converting it into excel is a better option. Regards Ankit

Hi there why do you need to put age when you already have the DOB and why do you also require a persons weight asking for height is sometimes ok as it can help sometime and also ask for number of dependent rather then exact bifurcation into a male/female. Else you judge it out whether that fulfills your requirement or your organization.
