The Instructions mentioned by you are pertaining to Chapter XIII which deals with personal hearing, determination of contribution, passing of speaking order u/s 45-A and recovery of contribution thereon.
I do not have copy of said instructions but I am attaching herewith ESI revenue manual in which there is a mention of the said instructions in page no. 224 and 225.
