I need a Decleration form i.e. form1 for ESIC as per 17 digit format.
Also I would like to know that, is ESIC accepting the printout/ xerox of the same at the time of submission of deleceration of new employee.

Dear Member, they accpet xerox copy of declaration form. u can get declaration form by ESIs websit.

Hi nirav, No. They are not accepting the xerox copy , cause they had return my froms which I had sumitted it on xerox. Secondly I didin’t found 17 digit format on ESIC web sight. Regards

There are two ways for submitting ESIC form of new employee:
1) Along with the declaration from fill the other form which is required for taking photograph and submit it to the local ESIC office.
2) Fill the declaration form in duplicate or take the copy of the filled declaration form and submit the same to the local ESIC office.
Both the ways are acceptable....

Attached herewith please find the esic new form. They will accept the declaration form xerox only if you take it in legal paper, it will not be accepted in a4 paper. do take it in legal paper size, and submit.
